The Loadable Card Program can be used for projects or programs that require participation of human subjects and compensation is offered as an incentive.  Types of projects include, but not limited to:

  • Research subject payments
  • Grant sponsored community outreach programs
  • Patient care

Through US Bank we currently have three loadable card programs available for departments to use:

  • Rewards Card – Single use card
  • Rewards Digital Card - Single use card
  • Focus Card – Reloadable card

Rewards Card FAQ's

I have a question about the Loadable Card Program. Who should I contact?

Accounts Payable Services manages the Loadable Card program. They can be reached at

loadablecard@uky.edu.

Are the US Bank Rewards cards reloadable?

The US Bank Rewards cards are non-reloadable. They are a single (one-time) load card.

What are the minimum and maximum amounts that can be loaded onto the Rewards card?

The minimum load amount is $10. The maximum load amount is $599 (see BPM E-9).

Does the Cardholder need to activate their card?

Yes. To activate, the cardholder can visit www.usbankrewardscard.com or call Cardholder Services at 855-274-9934 in the U.S. or 918-858-9782 outside the U.S. US Bank accepts relay calls. The card CANNOT be used until it has been activated and funded.

Can funds be reversed if an incorrect card or amount was loaded?

The department should immediately reach out to loadablecard@uky.edu and request that the funds be reversed. The funds can only be reversed if the cardholder has not used the card. Otherwise, no funds can be withdrawn from the card.

What if a participant reports their card as lost/stolen? Can we issue them a replacement card?

The cardholder must immediately contact US Bank Cardholder Services at 855-274-9934 in the U.S. or 918-858-9782 outside the U.S. to report a lost or stolen card. US Bank accepts relay calls. The project/department that issued the card should never issue a replacement card. The card is to be treated like a cash payment.

Does the card expire?

Yes, on the last day of the month and year printed on the front of the card.

The expiration date will be provided to the department each time a new batch of cards is assigned to their project.

What is the process to request additional US Bank Rewards cards?

The PI/Custodian can send their request for additional cards to loadablecard@uky.edu.

They should include their projects primary cost object (grant/WBS # or Cost Center) and the number of cards needed for the next quarter. Additionally, a current IRB approval letter or exemption letter must be on file or included with the request.

What is the process to fund the Rewards card?

What are the deadlines for funding a card the same day?

Card loads will occur Monday - Friday at 11am and 4pm (excluding holidays). Files received by these times will be loaded accordingly. Files that have errors/missing information will be delayed until the issue is resolved.

Focus Card FAQ's

Yes, the US Bank Rewards cards are reloadable.

What are the minimum and maximum amounts that can be loaded onto the Focus card?

The minimum load amount is $0.01. The maximum single load amount is $599 (see BPM E-9).

Does the Cardholder need to activate their card?

Yes. The cardholder can visit usbankfocus.com or download the U.S. Bank Mobile App to activate their card and choose their Personal Identification Number (PIN).  They can also call Cardholder Services at 888-863-0681. The card CANNOT be used until it has been registered, activated and funded.

Is card registration required?

Yes. The card must be registered to the participant before the initial card load. Please see the quick reference card (QRC) titled, “US Bank Focus Card Registration”.

Digital Rewards Card FAQs

I have a question about the Loadable Card program. Who should I contact?

What is a US Bank Digital Rewards card?

It is a digital card used for online purchases and is delivered via email. These cards can be redeemed at online retailers that accept Visa debit cards.

Are the Digital Rewards cards reloadable?

The Digital Rewards cards are non-reloadable. They are a single (one-time) load card.

What are the minimum and maximum amounts that can be loaded onto a Digital Rewards card?

The minimum load amount is $25. The maximum load amount is $599 (see BPM E-9-1).

Are there any fees associated with a Digital Rewards card?

US Bank charges a $1 fee per digital card. Accounts Payable Services will conduct a JV to charge the fee to the cost object associated with the project.

Does the participant need to activate and register their Digital Rewards card?

The participant must activate and register their digital card before using. They will receive an email from US Bank Digital Card notifying them that they have received a US Bank Digital Rewards card from the University of Kentucky. They should click on the link in the email to claim their card, which will direct them to the US Bank Digital Rewards Card website for activation and registration. The email link to activate their digital card expires in 30 calendar days.

Can funds be reversed if a digital card was sent to a wrong email address, or an incorrect amount was loaded?

Funds cannot be reversed if a digital card was sent to the wrong email address or if an incorrect amount was loaded. Once a funding file has been processed successfully, that action cannot be undone. The department should promptly contact loadablecard@uky.edu for further assistance.

What if a participant reports they never received the email from US Bank to claim their digital card?

First, verify with the participant that the email address listed in the funding file submitted to our team for processing is correct. If the email address is accurate, advise the participant to check their junk or spam folder. If they still cannot find the email, instruct them to contact US Bank Cardholder Services at 877-528-6413 to request a new email to be sent.

What should we do if a participant informs us that their Digital Rewards Card has been lost or stolen? Can we issue a replacement card?

Participants must call US Bank Cardholder Services at 877-528-6413 to report the digital card as lost or stolen. They will need to provide their name, Rewards Card number, original value, and transaction history. Please note that the project or department that issued the digital card should never issue a replacement card, as the digital card is to be treated like a cash payment.

Do US Bank Digital Rewards cards expire?

Yes, US Bank Digital Rewards cards expire two years after activation. The expiration date is indicated on the front of the card. Participants can view their Digital Rewards card online at https://rewards.mydigitalcard.com.

What is the process to fund a Digital Rewards card?
